Trauma Penetrasi Mata akibat Kail Pancing: Modified Back-out Technique Diikuti Tata Laksana Katarak Traumatik

Ocular trauma is an important cause of visual morbidity and may result in permanent visual impairment or blindness. Open-globe injury caused by fishhook penetration with an intraocular foreign body (IOFB) is uncommon but carries a high risk of serious complications. We report a case of a 31-year-old man who sustained a zone II penetrating ocular injury to the left eye caused by a fishhook, accompanied by hyphema and an IOFB. The patient underwent globe exploration and IOFB removal using the modified back-out technique without intraoperative complications. Postoperatively, the patient developed progressive visual impairment due to traumatic cataract associated with vitreous hemorrhage. After adequate control of ocular inflammation, phacoemulsification with posterior chamber intraocular lens implantation was performed. Postoperative visual acuity improved significantly to 6/7.5. Macular optical coherence tomography (OCT) revealed increased macular thickness in the left eye, consistent with post-traumatic changes without evidence of severe retinal involvement. This case demonstrates that prompt and comprehensive management, appropriate selection of an IOFB extraction technique, and staged treatment of post-traumatic complications can result in favorable visual outcomes in penetrating ocular trauma caused by a fishhook.
